چکیده

Results We describe the case of a 54-year-old man who underwent completion lobectomy for unanticipated hilar nodal metastasis reported on postoperative pathologic examination 1 month after right S8+9 segmentectomy for clinical T1N0 lung cancer. In the initial surgery, we used electrocautery without a stapler to divide the inter-segmental plane. The entire dissected inter-segmental plane was covered with absorbable mesh and fibrin glue. At re-thoracotomy, pleural adhesion at the inter-segmental plane was never observed although there were adhesions at parietal pleura not covered with fibrin glue.

Comparison of postoperative pulmonary function and air leakage between pleural closure vs. mesh-cover for intersegmental plane in segmentectomy

BACKGROUND To prevent postoperative air leakage after lung segmentectomy, we used two methods for the intersegmental plane: closing it by suturing the pleural edge (pleural closure), or opening it with coverage using polyglycolic acid mesh and fibrin glue (mesh-cover). The preserved forced expiratory volume in one second (FEV1) of each lobe and the postoperative air leakage were compared betwee...

Surgical Treatment of Pulmonary Aspergilloma

Introduction: Surgical approaches for the management of pulmonary aspergilloma have been accompanied with high levels of morbidity and mortality. However, these therapeutic options are still favored over other approaches for the treatment of Pulmonary Aspergilloma. In this study, we aimed to describe the characteristics of 30 patients with aspergilloma who referred to  Ghaem hospital between 20...

Transition from three-port vats pulmonary resections to uniportal vats pulmonary resections: a single center experience

Results 10 major pulmonary resections (9 lobectomies and 1 segmentectomy) and 28 wedge resections (7 pneumothorax, 16 lung cancers, 2 interstitial lung disease, 1 hamartoma, 1 tuberculoma, 1 fibrous tumor of pleura) were performed by a single surgeon. With wedge resections, mean postoperative hospital length of stay, chest tube duration, and operative duration were 3 days, 2.07 days, and 69 min...

Triple-layer sealing with absorptive mesh and fibrin glue is effective in preventing air leakage after segmentectomy: results from experiments and clinical study.

OBJECTIVES Fibrin glue in combination with polyglycolic acid (PGA) mesh is effective in preventing air leakage after segmentectomy, but we frequently experienced air leakage with single-layer application. To investigate improved usage, we compared the sealing effect among single-, double- and triple-layer PGA mesh and fibrin glue in both experimental and clinical segmentectomy. METHODS Ex viv...

Comparison of short-term effect of thoracoscopic segmentectomy and thoracoscopic lobectomy for the solitary pulmonary nodule and early-stage lung cancer

PURPOSE To compare the short-term effect of anatomic video-assisted thoracoscopic surgery (VATS) segmentectomy and VATS lobectomy. PATIENTS AND METHODS From January 2011 to December 2012, 21 patients underwent VATS segmentectomy and 61 underwent VATS lobectomy. Intraoperative blood loss, operating time, postoperative drainage time, length of hospital stay, postoperative complications, local r...
